Understanding Infertility

Infertility is a common condition, affecting approximately 1 in 6 couples worldwide. It is defined as the failure to achieve pregnancy after 12 months of regular, unprotected intercourse, or 6 months if the woman is aged 35 years or older. While the causes vary, they may include female factors such as ovulatory disorders, tubal disease, endometriosis and diminished ovarian reserve, male factors including sperm abnormalities and erectile dysfunction, a combination of both, or unexplained infertility.

The Role of the GP

GPs are often the first point of contact for people facing fertility challenges — and play a vital role in building trust, providing reassurance, and initiating early evaluation. Initial assessment should include a thorough history, physical examination, and lifestyle review.

EARLY INVESTIGATIONS

  • Semen analysis

  • Mid-luteal progesterone (ovulation check)

REFERRAL TRIGGERS

  • Female age >35 with >6 months trying

  • Abnormal semen parameters

  • History of pelvic surgery, PID, or endometriosis

  • Amenorrhea or irregular cycles

Supporting Patients

Infertility can bring grief, frustration, and stigma. GPs are well placed to provide meaningful emotional support by acknowledging feelings openly, using sensitive and inclusive language, normalising the experience, and referring to counselling or support groups when distress is high.

Lifestyle & Preconception Counselling

Encouraging patients to optimise their preconception health can make a meaningful difference to outcomes.

Key recommendations include:

  • Maintain a healthy BMI

  • Stop smoking and limit alcohol

  • Engage in regular exercise

  • Take folic acid supplementation

  • Reduce caffeine and stress

  • Encourage timed intercourse
